Team for Life - Choose Your Own Event!

Team For Life is a unique fundraising program. You pick the event. All your hard work benefits the Lazarex Cancer Foundation in helping us "Change the Face of Cancer."

You won't be alone when you become a Team for Life member.

Team For Life Annual Membership Pack ($35.00) includes:

• A Team For Life t-shirt and cap
• A personal Team For Life webpage that you can use to tell your story and track donations
• An online Team For Life Toolkit, with a personal fundraising coach and virtual online training coach

Participate in as many different events in as you want - all year long. All we ask is you raise a minimum of $500.00 on an annual basis.

The more you raise, the better the incentives are:

• $500 - $999: Team for Life sport duffel bag or travel mug (your choice)
• $1000 - $2499: All of the above + Team for Life carry-and-go lined weatherproof, fleece and nylon stadium blanket
• $2500 - $4999: All of the above + Team for Life PERSONALIZED microfiber jacket
• $5000 - $9999: All of the above + a $250.00 American Express gift card
• $10,000 and above: All the above + ONE WEEK LODGING AT AN EXCLUSIVE HAWAIIAN RESORT IN MAUI (airfare not included)
